What information is collected?

The information Pro-Wellness collects includes name, date of birth, address, telephone number, email address, emergency contact details, and brief medical history. Details of treatments are also noted and recorded in the best interest of the client.

Whilst you use this website we also receive information like your IP address, browser details and operating system data. This information is collected to improve your user experience.

When you purchase a service/gift voucher we also collect personal details such as name, email, recipients name (if gifting), postal address and brief medical/health notes. This information is used to identify you and provide the service you have requested.

How is the information obtained?

This information is collected during an initial contact and consultation as part of the treatment session. This information is also collected when you opt to fill in the content form online prior to your appointment. All information is recorded on to individual client treatment records, along with the client’s signature for informed consent to treatment.

Parents/Guardians are required to accompany any child under the age of 16 during treatments and must sign the informed consent forms on the behalf of the child undergoing treatment.

What is the purpose of storing this information?

The information you provide ensures your treatment progress is kept up to date so any changes can be made accordingly.

Client information will not be shared with any other party without the prior consent of the individual receiving treatment. The only instance that information will be shared is if a referral is required. This will be discussed with you and will only be conducted with your consent. This is for the benefit of your Wellness journey.

How do we store your information?

Client records are only stored in the treatments rooms when a client is due to attend an appointment. Your information cannot be accessed by any other individual or business.

In accordance with current guidance we are required to hold any personal treatment information for 10 years. For children under the age of 18, this information will be held for 10 years from their 18th birthday.

Our appointment booking service is provided by Acuity Scheduling. This company provides secure collection, processing and storage of personal data.

Payment

If you choose to pay for your massage with a debit or credit card your information will be passed to our payment processors - either PayPal for online card payments or takepayments for card payments in person.

We never store your credit card details, it is always processed by third-parties. It is encrypted through the Payment Card Industry Security Standard (PCI-DSS).

PayPal offers a service whereby you can pay for your appointment or gift voucher quickly using credit or debit card details previously stored with them. We can't see your stored card details or information.

PayPal adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, and American Express.
